But that range — 3 to 4 months — encompasses quite a bit of variation, and the factors that push results toward the shorter or longer end of that range are worth understanding before you schedule your appointment. What’s Actually Happening When Botox “Wears Off”

Botox (botulinum toxin type A) works by blocking the neurotransmitter acetylcholine at the neuromuscular junction — the connection between the nerve and the muscle it activates. When acetylcholine is blocked, the muscle can’t contract, and the overlying skin smooths. Over time, the nerve terminal sprouts new acetylcholine pathways that eventually bypass the block and restore muscle function. This process — not the breakdown of the toxin itself — is what determines how long results last.

Botox doesn’t “disappear” on a fixed timeline. It loses effectiveness as the nerve adapts. The process is gradual: most patients notice results beginning to soften around weeks 10–12, with full muscle activity returning by weeks 14–16 for most treatment areas.

Dysport vs. Botox: Dysport has a slightly faster onset but similar longevity to Botox in most patients. The choice between them is primarily about diffusion profile and the treatment area, not how long results last.

How to Make Botox Last Longer

The most evidence-supported approach: consistent treatment intervals. Waiting until your Botox has fully worn off before retreating allows the muscle to return to full strength — which means you start from zero with each appointment. Patients who maintain a consistent schedule (treating at 3–3.5 months, before results fully disappear) tend to see results improve over time rather than plateau. Zinc supplementation has some supporting evidence for extending Botox duration, though the effect is modest. Avoiding intense heat and exercise immediately after treatment (for 24–48 hours) reduces the risk of early diffusion.

What About Masseter Botox?

Botox for masseter reduction — to slim a square jaw or relieve teeth grinding — typically lasts 4–6 months initially, and longer with repeated treatment as the masseter muscle gradually reduces in size from disuse. This is one of the areas where the cumulative benefit of regular treatment is most pronounced.
